Job Title: Care Manager
Job Summary:
As a Care Manager, you will be responsible for managing the care of patients, ensuring they receive appropriate and timely medical attention. You will work with patients, families, and healthcare providers to develop and implement care plans, coordinate medical services, and monitor patient progress. You will also provide support and education to patients and families to help them manage their health and navigate the healthcare system.
Responsibilities:
- Conduct comprehensive assessments of patients to determine their medical needs, including reviewing medical history, performing physical exams, and ordering diagnostic tests.
- Develop and implement care plans tailored to each patient's individual needs, considering factors such as age, medical history, and overall health.
- Coordinate medical services, including scheduling appointments, arranging transportation, and communicating with healthcare providers to ensure seamless continuity of care.
- Monitor patient progress and adjust care plans as needed to achieve optimal health outcomes.
- Provide education and support to patients and families to help them manage their health, understand their treatment options, and navigate the healthcare system.
- Document patient interactions and maintain accurate and up-to-date clinical records and documentation.
- Collaborate with other healthcare providers, such as nurses, social workers, and physicians, to provide comprehensive and coordinated care to patients.
- Participate in ongoing education and training to stay current with new research, developments, and best practices in care management.
Requirements:
- Bachelor's or Master's degree in nursing, social work, or a related healthcare field.
- Current license or certification in the state of practice.
- Knowledge of healthcare systems and medical terminology.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and other healthcare providers.
- Strong attention to detail and ability to maintain accurate and up-to-date clinical records and documentation.
- Ability to prioritize tasks and manage time effectively in a fast-paced healthcare environment.
- Experience working in care management or a related field, preferably in a hospital or other healthcare setting.
- Demonstrated commitment to ongoing education and professional development.
