Job Summary:

 

We are seeking an experienced and compassionate Clinical Pharmacist to join our healthcare team. The Clinical Pharmacist will be responsible for providing pharmaceutical care to patients in a clinical setting, ensuring that medications are safe, effective, and appropriate for the patients' conditions. The ideal candidate should have excellent communication and interpersonal skills, be detail-oriented, and have a strong background in pharmacotherapy.

 

Responsibilities:

 

- Conduct patient assessments and recommend appropriate medications and treatment plans in collaboration with healthcare professionals.

- Monitor patients' medication therapy for safety and effectiveness.

- Provide medication education and counselling to patients and caregivers.

- Participate in medication reconciliation and medication therapy management programs.

- Review and interpret laboratory values and diagnostic tests to make medication therapy adjustments.

- Develop and implement protocols and guidelines for medication use.

- Work collaboratively with healthcare professionals to provide patient care.

- Provide drug information to healthcare professionals and patients.

- Document clinical interventions and patient progress notes.

- Maintain accurate records and reports of medication-related activities.

- Participate in quality improvement initiatives and research projects.

 

Requirements:

 

- Doctor of Pharmacy (PharmD) degree from an accredited school of pharmacy.

- Completion of a PGY1 residency program is preferred.

- Current state pharmacy license.

- Board certification in a relevant speciality area is preferred.

- Minimum of 2 years of clinical pharmacy experience.

- Strong knowledge of pharmacotherapy and drug interactions.

- Excellent communication, interpersonal, and teamwork skills.

- Ability to work in a fast-paced and dynamic environment.

- Strong attention to detail and accuracy.

- Proficiency in using computers and other relevant software applications.
