Job Summary:


We are looking for a dedicated and compassionate Community Health Worker to join our team. As a Community Health Worker, you will be responsible for assisting individuals and communities to adopt healthy behaviors and navigate the healthcare system. Your primary goal will be to help clients improve their health outcomes and well-being.




- Collaborate with healthcare providers and community organizations to develop and implement health education programs and interventions.

- Conduct outreach activities to identify and engage with individuals and families who are at-risk or experiencing health disparities.

- Conduct assessments to identify the needs, goals, and strengths of individuals and communities.

- Develop and maintain relationships with clients and provide ongoing support to help them achieve their health goals.

- Assist clients in navigating the healthcare system, including scheduling appointments, accessing health insurance, and obtaining necessary medications and medical equipment.

- Provide health education and coaching to individuals and groups to promote healthy behaviors, prevent chronic diseases, and manage existing health conditions.

- Maintain accurate and up-to-date client records and documentation.

- Participate in quality improvement activities to ensure that services are delivered in accordance with best practices and standards.

- Maintain confidentiality and adhere to ethical and professional standards.




- High school diploma or equivalent; Bachelor's degree in a related field preferred.

- Previous experience in a community health or social services setting preferred.

- Knowledge of health promotion and disease prevention strategies.

- Familiarity with healthcare systems, including insurance and public health programs.

- Excellent communication and interpersonal skills.

- Ability to work independently and as part of a team.

- Strong organizational skills and attention to detail.

- Proficient in Microsoft Office and electronic health record systems.
