Job Summary:

 

We are seeking a licensed and experienced Pharmacist to join our team. The Pharmacist will be responsible for dispensing medications, providing medication education and counseling to patients, and collaborating with healthcare professionals to ensure the safe and effective use of medications. The ideal candidate should have excellent communication and interpersonal skills, be detail-oriented, and have a strong background in pharmacotherapy.

 

Responsibilities:

 

- Review and interpret medication orders and prescriptions to ensure accuracy and appropriateness of medication therapy.

- Dispense medications to patients and ensure proper medication storage and labelling.

- Counsel patients on medication use, side effects, and interactions.

- Conduct medication therapy management and medication reconciliation programs.

- Monitor patients' medication therapy for safety and effectiveness.

- Collaborate with healthcare professionals to develop and implement medication therapy plans.

- Provide drug information to healthcare professionals and patients.

- Maintain accurate and complete medication records and reports.

- Participate in quality improvement initiatives and regulatory compliance programs.

- Order and maintain medication inventory.

 

Requirements:

 

- Bachelor's or Doctor of Pharmacy (PharmD) degree from an accredited school of pharmacy.

- Current state pharmacy license.

- Strong knowledge of pharmacotherapy and drug interactions.

- Excellent communication, interpersonal, and teamwork skills.

- Ability to work in a fast-paced and dynamic environment.

- Strong attention to detail and accuracy.

- Proficiency in using computers and other relevant software applications.
